How It Works
01 – Consultation
Once you’ve submitted the Contact form, we will review the information and reach out to schedule a consultation with our Program Director. Before the consultation, we will send you a referral packet to complete.
02 – Assessment
We will schedule an assessment with our Program Director.
03 – Tour
You will be welcomed to tour The Osiris House in-person or virtually.
04 – Apply
You will be invited to submit an application packet.
05 – Move In
You will have the option to select one of our available rooms that best suits the consumers needs. We will provide move-in instructions.
We plan to incorporate mindful-based practices for our residents, such as yoga and gardening, to aide them on their new journey to holistic living.
Admissions Criteria
Age Requirement
Consumers must be at least 18 years of age with a primary diagnosis of Developmental Disability and meet Medicaid Waiver Eligibility.
Behavior
May display self-abusive behavior, disruptive behavior (tantrum of yelling and screaming, inappropriate verbalizations, etc.), physical aggression, and property destruction which does not require the use of isolated time-out or mechanical restraints.
Documented Deficits
Men and women with documented deficits in at least two of the adaptive skill areas to include: Health status, communication, task learning skills, personal/self-care, mobile, behavior management and home and community living skills.
Psychological Evaluation
A psychological evaluation must have been completed prior to admission into the program (if applicable).
Physical Requirements
Consumers must be able to ambulate independently or with the use of a walker, cane and/or crutches and be able to travel up and down stairs with physical assistance from staff. May have medically related issues, which does not require the use of a wheelchair.
Secondary Diagnosis
Consumers may have a secondary diagnosis of mental illness.
Testing
A recent PPD must have been administered and indicate an absence of communicable disease with negative test results for tuberculosis.
